Honduras Service-Learning Trip
Veins Physician Application

Will you be traveling and participating in this service learning trip with any friends or family? Please note, the minimum age to volunteer is 14 years old.

GENERAL EXPERIENCE

Are you a new or returning participant?*
Do you speak Spanish?

VEINS

Can you perform lower extremity vein mapping?
Can you perform DVT exams?
Do you have a portable ultrasound to bring with you to Honduras?
Do you perform ultrasound guided foam sclerotherapy in your practice?
Do you have experience with using MOCA (ClariVein) in your practice?
Do you currently practice Phlebology?
Do you do your own ultrasound evaluations?

Please note, once your application is reviewed and approved, you will receive a second part to this application process.
